How to Track Your Symptoms: A Simple Daily Routine
A practical, two-minute daily routine for logging symptoms, severity and triggers so patterns become easy to see.
Why writing symptoms down helps
Memory is unreliable, especially for things that come and go. A short daily note turns vague impressions ("it's been bad lately") into facts you can look back on.
What to record each day
- The symptom, in your own words
- Severity on a 1–10 scale, rated the same way each time
- Time of day and how long it lasted
- Possible triggers: food, sleep, stress, activity, weather
- Any medication taken and whether it helped
Make it a habit
Pick one fixed moment — after dinner or before bed — and log even on good days. A "no symptoms today" entry is just as useful when you look for patterns.
